Description
The primary care Advanced Practice Provider for Gray Matters will be responsible for providing total health care management for an eligible population. This position has been created for primary care Advanced Practice Providers with functional medicine experience to or are able to:
- Spend the necessary time to listen to and take care of patients,
- Support patients who need help in addressing needed health related behavior change
- Manage chronic diseases with highly interactive tools and processes
- Focus on cognitive health and functional medicine approaches
This Advanced Practice Provider must be committed to putting the patient first by understanding the total person, their health risks, their goals and objectives and work collaboratively with the patient to engage them in maintaining or improving their brain health and overall health.
Responsibilities:
Strategic Health Care Planning and Alignment
- Providing primary care and oversight for an eligible population by reviewing all health risk factors identified through a comprehensive health evaluation or a health profile prior to their initial intake visit.
- Provide input and counsel on care plan for patient, health related improvements and family support to patient
- Measure patients monthly to evaluate progress and care plan needs
Health Care Delivery
- Reviews health evaluations on all eligible adult members and supports tailored action plans with them.
- Provides medical services to eligible population, responding to questions and concerns.
- Obtains, updates and reviews of medical histories.
- Orders and interprets reports, diagnostic tests, including radiologic exams, EKG’s, laboratory studies
- Identifies medical conditions contributing to poor health
- Determines, executes and manages treatment plan
- Manages supplements and medications
- Instructs and counsels patients regarding health conditions, health maintenance and prescribed treatment plan
- Coaches and counsels patient to support them in complying with care treatment plan
- Completes accurate and thorough documentation of all patient encounters in EMR system
- Understands and implements medical protocols and clinical standards.
- Develops relationships with the community of providers to oversee all the clinical health of the eligible members
- Utilizes technology to insure efficient management of patients
- Recommends changes in all clinical areas and/or in general areas to improve patient brain health and wellness by doing an assessment and evaluation quarterly
- Manages chronic diseases with highly interactive care to facilitate successful patient outcomes (education, nurse support and tools provided)
Support patients in changing health-related behavior
- Provides educational support
- Listens to and understands the patient’s values and needs
- Uses strategic and effective coaching techniques and guidelines
- Supports and coordinates goals for each patient to be monitored and tracked
- Supports patients in changing health related behaviors (education, nurse support and tools provided)
- Participates in patient education sessions among targeted populations with prevalent health risk factors, such as diet and exercise

Schedule
The parties agree Grey Matters shall set the schedule for Advanced Practice Provider’s provision of Services in accordance with the needs of health center patients. This position requires assessment prior to initial visits and follow up within 24 – 48 hours after initial assessment to answer any questions and support care plan. In addition, will be responsible for doing quarterly assessment of all Grey Matter patients to measure progress.
Requirements
- Experienced in either Internal Medicine or Family Practice
- Physician Assistant must be certified by NCCPA, have a valid and current state license, and federal DEA number. Nurse Practitioner’s must be certified by AANP, have a valid and current license, and federal DEA number.
- A commitment to working collaboratively with the clinical team and patient to improve their health through behavior change.
- A commitment to continuously improve the health and wellness of each eligible member
- Good leadership skills and communication skills
- Experience with electronic medical records
