Opportunity InformationSutter West Bay Medical Group (SWBMG) is seeking a full-time Nurse Practitioner to join our Cardiology team at 1580 Valencia Street in San Francisco, California. This role supports two cardiologists in a growing outpatient practice and is essential to improving patient access and care continuity.

About the Opportunity
The NP will manage follow-up visits for established patients, allowing cardiologists to focus on new referrals and complex cases. The NP will also support transitional care efforts and provide timely post-discharge visits for patients with heart failure. A key part of this role includes educating patients on cardiovascular conditions, treatment plans, and lifestyle modifications to support long-term health.
Key Responsibilities:
• Conduct follow-up visits for established cardiology patients
• Support transitional care management through post-discharge visits
• Provide patient education and promote medication adherence
• Collaborate with cardiologists and care teams to ensure continuity of care
• Document patient encounters in Epic

Requirements
Education:
• Graduate of an accredited Nurse Practitioner (NP) program
Licensure & Certifications:
• Current California NP license
• DEA Controlled Substance Registration Certificate (CSRC)
• Current BLS and ACLS certification
Experience:
• Experience in cardiology or internal medicine preferred
• New graduates with a strong interest in cardiovascular care are welcome to apply

Sutter West Bay Medical Group (SWBMG) is a premier multi-specialty medical group made up of over 200 physicians practicing in the greater San Francisco service area. SWBMG works in partnership with Sutter Pacific Medical Foundation (SPMF), a non-profit organization providing care through its affiliation with SWBMG. Physicians work as a team to provide patient-focused care that encourages a sense of teamwork. SPMF provides physicians with an administrative infrastructure, allowing physicians to focus on patient care.
