Horizon Health in Martin, SD is seeking a full-time Nurse Practitioner or Physician Assistant to provide direct patient care for all patients. The organization is looking for a highly professional individual with strong communication skills who is committed to coordinating all aspects of primary care for the patients served. Successful candidates must be licensed or be able to successfully obtain licensure in the State of South Dakota. Certification from an accredited organization is needed or must be obtained prior to employment.
Key Responsibilities:
Provides primary health care to patients, according to established evidence-based guidelines of clinical practice.
Ability to lead care teams to provide high quality healthcare in accordance with Horizon Quality Improvement Plan and meet established goals and benchmarks.
Management of long-term care for acute and chronically ill patients, including care coordination and development of care plans and self-management plans.

Requirements
Essential Duties and Responsibilities
The following key duties and responsibilities are identified for the Advanced Practice Provider.
Other duties may be assigned.
- Provides primary health care to patients, according to established evidence-based
guidelines of clinical practice and the current health care plan. - Supervises and consults with precept medical students and Advanced Practice Providers, as directed.
- Strong ability to utilize the Electronic Health Record (EHR) for patient tracking and
documentation, including management of patients in need of preventative or chronic care services. - Maintain and complete patient documentation within Horizon Health’s policies and procedures.
- Ability to lead care teams to provide high quality healthcare in accordance with Horizon
Quality Improvement Plan and meet established goals and benchmarks. - Serve as a resource for clinical staff. Training and assigning members of the care team to support
and manage the patient population. - Serves as a role model for effective teamwork among all staff and actively works to
maintain positive morals, attitude and professional interactions. Ability to work as a team
within clinic setting and participate and be engaged in Horizon Health’s established work
groups, committees, etc. - Assist the Chief Medical Officer and Leadership Team in revision and implementation of
established evidence-based guidelines of clinical practice and the current health care plan. - Management of long-term care for acute and chronically ill patients, including care
coordination and development of care plans and self-management plans. - Acquires, maintains hospital privileges, and provides care at local hospital, if required.
- Establish and maintains rapport with community agencies and organizational
partnerships. - If needed, provide services to health center and non-health center patients in the
emergency room or afterhours. - In accordance with the FTCA (Federal Tort Claims Act) Malpractice Insurance Scope of
Practice guidelines issued in September 1995, within the scope of section 6.6 (d), the
following activities are approved for FTCA coverage: school-based clinics, health fairs,
immunization campaigns, migrant camp outreach, homeless outreach services, and
periodic hospital call if required for privileges and formal after hour’s coverage
arrangements. - Appropriate training, direction, and guidance will be offered within the employee’s scope
of practice for the employee to competently perform duties assigned under the discretion
of the Chief Medical Officer and Leadership Team. - Maintain the confidentiality and safety of patient health information by complying with
organizational HIPAA Privacy and Security policies. - Collaborate with care team during team meetings, huddles and staff meetings as an
active, positive participant of clinic team. - Ensure clinic complies with OSHA regulations and infection control standards
while working to maintain operations of equipment. - Engage in emergency preparedness procedures within the clinic, including
maintenance of CPR certification. - Maintain supply inventory in clinic setting, if requested.
- Other duties as assigned by supervisor.
