Position Overview

The Registered Practical Nurse (RPN) reports to the Director of Care and plays a key role in coordinating and overseeing all aspects of resident-centered care. This position provides leadership, supervision, and guidance to the healthcare team, advocates for residents and families, and supports the Home’s mission, goals, and values.
Key Responsibilities:
- Develops and updates individualized care plans based on residents’ changing needs
- Collaborates with healthcare partners, external agencies, and families
- Builds and maintains strong relationships with medical staff, coworkers, residents, and the public
- Provides supervision on the unit and assumes charge responsibilities when needed
- Monitors performance and completes evaluations for PSWs and RPNs
- Assigns, directs, and assesses the work of PSWs
- Uses strong leadership and problem-solving skills when acting in charge of the Home
- Ensures care delivery aligns with the Resident Bill of Rights
- Understands and supports the Home’s quality indicators, including those publicly reported
- Participates in quality improvement initiatives and meets performance targets
- Orders and manages nursing supplies and equipment effectively
- Listens to and addresses resident or family concerns in line with Home policies
- Performs unit rounds at shift start and end
- Processes physician orders, administers medications/treatments, and reports/document errors in accordance with policies and College of Nurses standards
- Maintains accurate, compliant nursing documentation
- Assesses health changes and communicates updates to the SDM/POA and interdisciplinary team
- Completes RAI-MDS and other required assessments, and collaborates on care plans
- Understands mandatory reporting requirements for critical incidents and follows Home policies
- Performs additional duties as required or listed in the Job Task Inventory
Requirements
Qualifications:
- Current Certificate of Competence with the College of Nurses of Ontario as a Registered Practical Nurse (or an equivalent registration under Bill 60 provisions from another Canadian jurisdiction)
- Basic CPR certification
- Experience in long-term care is an asset
- Strong communication skills and ability to collaborate across disciplines
- RAI-MDS knowledge (AIS certification required within one year if not already obtained)
- Demonstrated ability to engage residents positively and foster meaningful interactions
- Ability to meet all legal, safety, and rights-based obligations
- Vulnerable Sector Check (or willingness to obtain one)
- TB test results (within 6 months, or within 14 days of hire) or chest X-ray (within 1 year), as per Public Health guidelines
- Two supervisory references
- Proof of required vaccinations; staying “up to date” is strongly encouraged
- French or Italian language skills are an asset
