Company History

Outreach Community Health Centers was originally established in 1979 as a volunteer-driven initiative called Health Care for the Homeless, created to provide medical and supportive services to individuals experiencing homelessness in Milwaukee. The organization was formally incorporated in 1980 and began delivering direct healthcare services in 1984 through a national demonstration project funded by the Robert Wood Johnson Foundation and the Pew Charitable Trusts.

Over the following decades, the organization expanded its services beyond homeless outreach to include primary care, behavioral health, and community support programs for underserved and low-income populations. In 2011, the organization rebranded as Outreach Community Health Centers to reflect its broader mission of serving the wider community while maintaining its commitment to vulnerable populations.

Today, Outreach Community Health Centers operates multiple service locations and provides comprehensive healthcare services—including primary care, behavioral health, dental services, and outreach programs—serving thousands of patients throughout the Greater Milwaukee area.
